Good resource for your HOP
A good resource and toolkit to understand HOP and also for understanding Learning Teams in more detail. The books also lays out the prerequisites of being a good LT facilitator. I recommend to anyone wanting to step change safety in their organisation

Terrific resource for learning teams
Well written, and easy to use summary on implementing Learning Teams - a must read if you are planning on a move to this approach. The intro is by Todd Conklin, considered by many as the Godfather of Learning Teams, sets the tone for a very user-friendly work.
